Summit Elder Care A Program of All-inclusive Care for the Elderly (PACE), Summit ElderCare is a medical, insurance, and social support system which helps aging adults to remain at home and avoid a nursing home placement.

CMHA operates an array of programs ranging from homelessness prevention to affordable homeownership and services for elder homeowners. Evercare Evercare Senior Care Options offers Medicare and Medicaid health care services to MassHealth (Medicaid) members, aged 65 or older, who have chosen to participate in the program. This plan is available in the following counties: Bristol, Essex, Hampden, Middlesex, Norfolk, Plymouth, Suffolk, and Worcester.


Senior Whole Health Senior Whole Health (SWH) is a new health care plan for people age 65 years or older who qualify for MassHealth Standard and live in the Senior Whole Health service area. Eligible persons may also have Medicare. Service area: Worcester, Middlesex, Essex, Norfolk, Suffolk, Bristol, and Plymouth.
